Anatomy of left ventricular outflow tract.

چکیده

The left ventricular outflow tract is here considered to be that region of the left ventricle that lies between the anterior cusp ofthe mitral valve and the ventricular septum. The ventricular septum lies at an angle of45 degrees to the median plane. The 'anterior' wall of the outflow tract is therefore anteromedial and the 'posterior' wall is posterolateral. In the anterior wall are both the muscular and membranous parts of the ventricular septum. The outflow septal myocardium is smooth-walled. At the junction of the muscular and membranous parts of the septum lies the atrioventricular bundle. The right face of the membranous septum gives attachment to the medial wall of the right atrium and also to a small part of the septal cusp of the tricuspid valve. The membranous septum is therefore partly atrioventricular. At its upper end the membranous septum is continuous with the right lateral wall of the root of the aorta. The root of the aorta passes to the right and upwards and the membranous septum separates the aorta from the right ventricle. The posterior wall of the outflow tract is formed not only by the anterior mitral cusp but also by the 'intervalvar septum' and, in its upper part, by a curtain formed by the fusion of the anterior and posterior mitral cusps.

The outflow tract of the left ventricle.

Intensive study of the infundibulum in congenital and acquired lesions has led to a much better appreciation of the anatomy and function of the right ventricular outflow tract in health and disease. The rapidly spreading practice of replacing severely damaged aortic and mitral valves, together with current interest in subaortic stenosis (Braunwald et al., 1964) have focused attention on the out...

Obstructive Lesions of the Left Ventricular Outflow Tract

FOR YEARS successful surgical treatment of obstructive lesions of the left ventricular outflow tract lagged behind the often brilliant results obtained for more than 15 years in correction of inflow obstruction to the left ventricle at the mitral valve. Closed operations for relief of aortic valve obstruction gave early promise, but results were unpredictable, relief was usually temporary, and ...

Reoperation for left ventricular outflow tract obstruction after repair of atrioventricular septal.

Left ventricular outflow tract obstruction (LVOTO) is an important source of morbidity and mortality after repair of atrioventricular septal defect (AVSD). The intrinsic anatomy of the left ventricular outflow tract in AVSD is complex and predisposes to the development of LVOTO. LVOTO after repair of AVSD usually involves multiple levels and sources of obstruction, and surgical intervention mus...

Rastelli procedure for transposition of the great arteries, ventricular septal defect, and left ventricular outflow tract obstruction. Early and late results in 41 patients (1971 to 1978).

Forty-one children with transposition of the great arteries, ventricular septal defect, and left ventricular outflow tract obstruction underwent a Rastelli operation between 1971 and 1978. A homograft valve preserved in an antibiotic solution and extended with A dacron tube was the conduit of choice. Alternatively, conduits with porcine heterografts or valves constructed from calf pericardium w...

Discovery of a Symptomatic Left Anomalous Coronary Artery from the Opposite Sinus and Postoperative Considerations

This is the case of an 18 year old active duty soldier with symptoms of exertional chest pressure and syncope who was found to have anomalous origin of the left main coronary artery (LMCA) from the right coronary cusp (RCC) traveling partially between the great vessels before taking a septal approach between the left ventricular outflow tract (LVOT) and the right ventricular outflow tract (RVOT...

Myectomy plus Alfieri technique for outflow tract obstruction in hypertrophic cardiomyopathy.

A 56-year-old man presented with breathlessness and chest pain. Echocardiography revealed 14-mm septal hypertrophy, complete systolic anterior motion of the anterior mitral valve leaflet, severe central mitral regurgitation, and a left ventricular outflow tract gradient of 40 mm Hg increasing to 56 mm Hg on Valsalva. A diagnosis of hypertrophic cardiomyopathy was made. Despite medical therapy, ...
